Developed with Latin American identity specialist Extrimian, the infrastructure has been live since June 10, 2026. The deployment places a nationwide chain of institutional trust on a public blockchain without exposing patient records. Each authority holds a verifiable identity, while INCUCAI operates as the root that accredits the provincial agencies, mirroring the hierarchy used to coordinate donation, procurement, and transplantation procedures across the country through hospitals and transplant centers nationwide.
Blockchain verification remains invisible to patients and officials
The system lets accredited officials electronically sign documents and notarize immutable, timestamped fingerprints on IOTA. Only cryptographic hashes of documents and process steps reach the ledger, never the underlying medical or personal information. INCUCAI is using blockchain as a verification layer rather than a repository for sensitive health data. Doctors, agencies, and families can confirm that a record existed and remained unchanged while its contents stay private. The full signing, notarization, and verification flow has been demonstrated, addressing a process where misplaced forms, forged signatures, or unverifiable authorizations can create dangerous delays during urgent coordination.
The architecture combines IOTA Identity, Hierarchies, Notarization, and Gas Station. Decentralized identifiers follow W3C standards, and INCUCAI’s onchain identity is cryptographically linked to its official government domain. The hierarchy allows the national authority to grant, suspend, or restore provincial accreditation, while Gas Station sponsors transaction costs. Officials and citizens can use the blockchain-backed system without holding tokens or managing network fees. Critical components also run on Argentina’s state-owned ARSAT infrastructure, preserving national control. IOTA Foundation said it independently verified all 24 provincial identities, federation links, and controlling entities through public endpoints and Explorer data directly.
Official adoption began in June through Resolution 199/2026, extending electronic signatures across processes managed through SINTRA. IncucaiID, the ecosystem’s official wallet, lets patients, healthcare professionals, and other authorized users receive verifiable credentials, prove identity, and sign documents. The rollout starts with heart and lung waiting-list registration before expanding across additional lists and SINTRA workflows.
